Evaluation of Retinal Nerve Fiber Layer Thickness in Swollen Optic Disc

5Dokuz Eylül Üniversitesi Tıp Fakültesi Göz Hastalıkları A.D., İzmir, Prof. Dr. Purpose: To evaluate the retinal nerve fiber layer thickness (RNFLT) in optic disc swelling by using Heidelberg Retina Tomography (HRT) and Optical Coherence Tomography(OCT).
Materials and Methods: 40 patients with swollen optic disc and 41 healthy subjects were enrolled in the study. Ophthalmological examination, Mean RNFLT measurements with HRT III and OCT were performed.
Results: There were 17 women and 23 men. The mean age was 41.3±17.3 years in the patient group and 47.8±10.1 years in the control group. The involved eye in unilateral cases and more seriously affected eye in bilateral cases were taken into consideration. There were 11 pseudotumor cerebri (PTC), 11 nonarteritic anterior ischemic optic neuropathy (NAION), 8 optic disc drusen (ODD), 6 central retinal vein occlussion (CRVO) and 4 optic neuritis (ON). The mean RNFLT was 176.10 μm, 251.47 μm in patient group and 98.85 μm, 275.60 μm in control group with OCT and HRT. There was a statistically significant positive correlation between two methods in patient group. However, there was no correlation in control group. RNFLT was found significantly higher in control group than in NAION subgroup with HRT. RNFLT was found significantly higher in all groups than in control group and significantly higher in PTC group than ODD group with OCT.
Conclusion: Although, a positive correlation of RNFLT between HRT and OCT was found in patient group; much thicker measurements were obtained by using HRT. The results of our study showed that OCT for detecting RNFLT may be more useful in the differential diagnosis of optic disc swelling.
