The Evaluation of Scleral Buckling, Pars Plana Vitrectomy and Pars Plana Vitrectomy with Scleral Buckling in the Management of Primary Rhegmatogenous Retinal Detachment

Dr. Purpose: The aim of this study was to evaluate the results of scleral buckling (SB), pars plana vitrectomy (PPV) and combined scleral buckling and pars plana vitrectomy (SB+PPV) surgeries for the treatment of rhegmatogenous retinal detachment.
Material and Methods: This study included cases (101 cases) who received surgical treatment due to primary rhegmatogenous retinal detachment in our clinic, between April 2006 and April 2010. According to surgery techniques, the cases were evaluated retrospectively and divided into three groups as SB, PPV and SB+PPV. The groups were evaluated for anatomical and functional success and encountered complications.
Results: When the cases were evaluated with regard to their related factors, the ratios of myopia, pseudophakia, trauma, and aphakia were determined around 26.7%, 39.6%, 14.9%, and 3% respectively. After the first surgeries, anatomic success rates were determined about 54.9% in SB, 80.6% in PPV, 85.7% in SB+PPV group. After recurrent surgeries, anatomic success rates were obtained around 88.2% in SB, 91.7% in PPV, 100% in SB+PPV group. Functional success rates were found out 76.5% in SB, 72.2% in PPV, 78.5% in SB+PPV group. Cataract was the most frequent complication in PPV (52.6%) and SB+PPV groups (50%). The most frequent complication in SB group was determined as the recurrence of retinal detachment (45.1%).
Conclusion: In our opinion the best results can be obtained when the appropriate surgical method was performed carefully with using of appropriate material for the treatment of rhegmatogenous retinal detachment. Keywords : Pars plana vitrectomy, rhegmatogenous retinal detachment, scleral buckling
