Factors Affecting Visual Acuity Among Patients with Pars Plana Vitrectomy Due to Nucleus Dislocation During Phacoemulsification Surgery

3Ankara Atatürk Eğitim ve Araştırma Hastanesi, 1. Göz Kliniği, Ankara, Prof. Dr. Purpose: To investigate the factors affecting visual acuity among patients underwent pars plana vitrectomy (PPV) for treatment of nucleus dislocation during phacoemulsification surgery.
Materials and Methods: We enrolled a consecutive subset of 29 patients who referred to our clinic for PPV due to nucleus dislocation. Best corrected visual acuities were measured using Snellen visual acuity chart preoperatively and postoperatively at one week and 3 months, and they were converted to logarithm of the minimal angle of resolution (logMAR) visual acuity. The time interval between cataract surgery&PPV, being pseudophakic or aphakic before and after operation, applied surgical technique, the use of perfluorocarbon liquids (PFC) were analysed as factors affecting the final visual acuity.
Results: The mean values for preoperative, postoperative 1st week and postoperative 3rd month visual acuities were (mean±standart deviation) 2.31±0.21 (range; min:0.1-max:3.0) logMAR, 1.84±0.94 logMAR (min:0.52-max:3.0) and median visual acuity at the end of 3rd month was 1.97±0.15 logMAR (min:0-max:3.0) respectively. Visual acuity values were similar between preoperative and postoperative 1st month (p=0.069), however significant difference was observed between preoperative and postoperative 3rd month (p=0.001) visual acuities. When vitrectomy with 20 g and 23 g and the use of PFC considered, there was no significant difference between the visual acuities. Visual acuity at 1st week was lower in aphakic patients compared to pseudophakic patients (p=0.004); however visual acuity values were similar between preoperative and postoperative 3rd month. Visual acuity at 1st week was better in patients with intraocular lens (IOL) after vitrectomy when compared to aphakic patients (p=0.001); however visual acuity values were similar at postoperative 3rd month evaluations.
Conclusion: IOL implantation immediately after phacoemulsification surgery or in the same session with vitrectomy are associated with better rehabilitation of patients in early period. Final visual acuity was not affected from the factors including the time interval between cataract extraction and vitrectomy, applied surgical technique and the use of PFC.
