TREATMENT OF DROPPED INTRAOCULAR LENSES WITH PARS PLANA VITRECTOMY

Purpose: To evaluate the results and complications of dropped intraocular lenses (IOL) during cataract surgery or follow-up period treated with pars plana vitrectomy.Methods: Fifteen eyes with dislocated IOL treated in our clinic between 1996-1999 were studied. The ethiology of the dislocated IOL were posterior capsul perforation in 13 eyes (86.6%) and early postoperative trauma in 2 eyes (13.3%). In 10 eyes IOL reposition ( 1 eye endocapsular, 7 eyes sulcus implantation and 2 eyes one haptic anterior capsular the other haptic scleral fixation) and in 5 cases IOL exchange was performed.
Results: After a mean follow up period of 7.5 month, a total of 11 eyes (72%) had equal to or higher than 5/10 snellen visual aquity. But the same value of visual acuity improvement was 90% (9/10) in IOL reposition group and 40%(2/5) in IOL exchange group. Postoperative complications were cystoid macular edema (CME) in 4 eyes (26.6%), retinal detatchment (RD) in 1 eyes (6.6%), corneal edema in 2 eyes (13.3%) and IOL desantralisation in 2 eyes (13.3%). CME and retinal detatchment complications were all in IOL exchange group, and they were the cause of bad visual prognosis (<5/10).
Conclusion: Apart from the good anatomic results in both group, when we consider worse visual result in the IOL exchange group, we suggest to preserve the remnant of lens capsule as much as possible to be able to replace IOL instead of exchange with scleral fixation IOL.
