PARS PLANA VITRECTOMY AND PARS PLANA TUBE IMPLANTATION IN INTRACTABLE GLAUCOMA

A planned surgical technic to controll the intraocular pressure in 15 eyes of 15 cases with intractable glaucoma is presented. Patients wilh neovascular glaucoma were excluded. Mean age of the patients was 45.93 (6-70) years and mean follow-up period was 21.26 months (3-59). following the three port total pars plana vitrectomy, the tube was inserted into the vitreous cavity via upper temporal sclerostomy. Then, dura mater patch was sutured covering the sclerotomy site and the area between sclcrotomy site and seton disk. In 13 eyes setons with disk were used and only in one case a tube with encircling material consistent with Shocket surgery was preferred. One case als underwent partial penetrating keratoplasty at the same session. Four eyes had penetrating keratoplasty later on.The most severe complications were intravitreal hemorrhage, choroideal detachment and early hypotony without any long-term serious sequale. In one case, retinal detachment was seen 3 months after surgery, 12 of 15 eyes maintained good glaucoma control. Only two eyes repuircd additional anliglaucomatous drops and one pars plana tube insertion can be a surgical alternative for patients with intractable glaucoma.
