Pars Plana Vitrectomy- Partially Penetrating Keratoplasty And Iol Exchange In Pseudophakic Bullous Keratopathy

Results of pars plana vitrectomy (PPV) combined with partial penetrating keratoplastiy (PPK) and intraocular lens (IOL) exchange in 11 cases with pseudophakic bullous keratopathy (PBK) is presented. 10 cases had anterior chamber IOL (AC-IOL) and only one case had posterior chamber IOL (PC-IOL) preoperatively. All AC-IOL's were explanted and reimplantation was performed using new AC-IOLs in four cases and scleral fixated PC-IOLs in 6 cases. Iridoplasty and/or gonioplasty was performed in 7 cases.
Postoperative visual acuities (range: counting fingers at 5 meters to 6/10) were better than preoperative levels (range: hand movement to counting fingers at 3 meters) in all of the cases. All grafts were clear with the mean follow-up time of 9 months (range: 2-25 months).
PPV provides total vitrectomy, removal of all capsulocortical remnants and lysis of all synechia. PPV combined with PPK and IOL exchange is a safe and effective procedures in cases with PBK and vitreous opacities.
