Surgical Alternatives in Retinal Detachments Seen after Cataract Surgery

Materials and Methods: Two hundred seven eyes of 203 patients with aphakic or pseudophakic retinal detachment who underwent surgery between 1995-2005 were included in this study. The cases were reviewed retrospectively in terms of surgical technique, anatomical and functional results.
Results: Ninety eyes were aphakic and 117 were pseudophakic. The mean time between cataract surgery and the development of retinal detachment was 40.14±59.72 months. In 141 eyes (68.1%) a retinal break was found but in 66 (31.9%) no break was detected. 171 eyes (82.6%) operated by scleral buckling, 22 eyes (10.6%) by pars plana vitrectomy, 8 eyes (3.9%) by combined pars plana vitrectomy and scleral buckling and 6 eyes (2.9%) by pneumatic retinopexy. The anatomical reattachment rates were 72.5%, 68.2%, 87.5% and 50% respectively after one procedure. The final anatomical success rate was 93.7%. The mean preoperative visual acuity was 2.49±0.79 (logMAR) and the mean postoperative visual acuity was 1.27±0.75. The final visual acuity was 1.2±0.74 in the scleral bucking group, 2.08±0.96 in the pars plana vitrectomy group, 1.8±0.6 in the combined pars plana vitrectomy and scleral buckling group and 0.6±0.58 in the pneumatic retinopexy group
Conclusion: Anatomical and functional success rates were satisfactory with either scleral buckling or vitreoretinal surgery fort he retinal detachments seen after cataract surgery. Keywords : Retinal detachment, aphakia, pseudophakia, scleral buckling, pars plana vitrectomy
