Using Tissue Glue For the Closure of Scleral Sites in Vitreoretinal Surgery

4Ankara Üniversitesi Tıp Fakültesi Göz A.D., Ankara, Prof. Dr. Purpose: To assess the surgical outcome of the use of tissue glue to close sclerotomy sites and/or conjunctival openings and the appearance of ultrasound biomicroscopy (UBM) of the sclerotomy sites in 20- and 25-gauge vitrectomy.
Materials and Methods: Eleven eyes of 11 patients were included in this study. After 6 eyes underwent pars plana vitrectomy with the 20-gauge system, scleral and conjunctival openings were closed by using tissue glue (Tisseel, Baxter, AG). While vitreoretinal surgery was performed by 25-G vitrectomy, wound leakage was observed at the sclerotomy sites in 5 eyes at the end of the surgical procedure and sclerotomy sites were closed by using tissue glue. The indications for vitreoretinal surgery were diabetic vitreous hemorrhage (4 eyes), retinal detachment (6 eyes), and silicone oil removal (1 eye). Silicone oil tamponade (5000 cs) was used in 2 eyes and 20% C3F8 tamponade in 6 eyes. Detailed ophthalmic examinations were performed 1, 3, 7, 14, 30, and 60 days after surgery. The sclerotomy sites were evaluated with ultrasound biomicroscopy at the second and fourth week postoperatively.
Results: No wound leak was observed at the end of the surgical procedure or during the follow-up period. Conjunctival detachment at conjunctival opening sites and pronounced intraocular inflammation were not observed. Abnormal fibrous ingrowth was not detected at the sclerotomy sites by means of UBM. Hypotony and/or hypertony were not observed during the follow-up in any eyes.
Conclusions: Our results demonstrated the efficacy of tissue glue for closing entry sites and conjunctival openings. Tissue glue had no adverse effects on ocular tissue and might serve as an effective substitute to avoid scleral wound closure and suture related complications in different types of vitreoretinal surgery.
