Complicated Retinal Detachments Treated with Pars Plana Vitrectomy and Large Relaxing Retinotomy

4S.B. Beyoğlu Göz Eğitim ve Araştırma Hastanesi Klinik Şefi, İstanbul, Prof. Dr. Purpose: To evaluate the anatomical and functional results in patients with complicated retinal detachment who were treated with pars plana vitrectomy and large relaxing retinotomy.
Materials and Methods: Twenty-six eyes of 26 patients who underwent pars plana vitrectomy with retinotomy larger than 90 degrees because of complicated retinal detachment (RD) were included in this study. The indications of pars plana vitrectomy in our study were traumatic RD in 9 eyes (35%), rhegmatogenous RD in 6 eyes (23%), diabetic tractional RD in 4 eyes (15%), pseudophakic RD in 3 eyes (11.5%), recurrent RD in 3 eyes (11.5%), and tractional RD due to branch retinal vein occlusion in 1 eye (4%). Anatomical and functional success rates were determined by evaluating the patient files retrospectively.
Results: The complete reattachment rate after one operation was 50%, with a final complete reattachment rate of 65%. Visual acuity improved in 10 eyes (40%), remained the same in 12 eyes (48%), and became worse in 3 eyes (12%). Visual acuity could not be evaluated in a 3-year-old patient. Phthisis bulbi occurred in 4 eyes (15%).
Conclusion: Limited postoperative anatomic and functional success can be obtained following large relaxing retinotomy in complex retinal detachment as it may cause severe complications. Recurrent retinal detachments due to reproliferation seemed to be the main reason for the anatomical and functional failure.
