Epstein-Barr Virus Related Retrobulbar Neuritis with Spontaneous Recovery

3Ankara Atatürk Eğitim ve Araştırma Hastanesi, Göz Hastalıkları, Ankara, Prof. Dr. The ophthalmological examination of a 24-year-old female who came to our clinic with a complaint of decrement in vision, which started 15 days ago and advanced quickly, revealed that the patient had a corrected visual acuity of 4/10 in the right eye and 6/10 in the left eye. In the fundus examination, it was found that both eyes had multiple soft exudations in the posterior pole. The direct light reflex was observed to be bilaterally weak positive. Ishihara test revealed a color vision of 4/12 in the right eye and 3/12 in the left eye. Fluorescein angiography in the later phase showed minimal leakage from the optic nerve head. After the tests for EBV infection, the patient was found to be VCA IgM and VCA IgG positive and was diagnosed with retrobulbar neuritis associated with EBV infection. The patient was followed up without any medication. During the follow-up appointment 15 days later, it was found that the visual acuity on both eyes had increased up to 10/10, the exudations in the retina had noticeably decreased and the leakage that was observed previously in the fluorescein angiography had disappeared. Although there are different approaches for the treatment of optic neuritis associated with EBV infection, some of these patients can be observed to recover spontaneously. Therefore, the risks and benefits of the treatment options in this type of cases need to be evaluated carefully. Keywords : EBV, retrobulbar neuritis
