Vitreoretinal Surgery Outcomes in Patients with Intraocular Foreign Body

3S.B. İstanbul Eğitim ve Araştırma Hastanesi Göz Kliniği, İstanbul, Doç. Dr. Purpose: To evaluate the anatomic and functional outcomes of pars plana vitrectomy and intraocular foreign body (IOFB) removal in patients presenting with open-globe injuries due to lacerations by retained IOFBs.
Material and Methods: 55 male (91.6%) and 5 female (8.4%) patients who were diagnosed with IOFB were examined retrospectively in our clinic between March 2004-January 2010. Patients were evaluated according to complications and the best corrected visual acuity (BVCA) post-operatively. Pars plana lensectomy or phacoemulcification surgery was performed in 32 (53.3%) patients that had traumatic cataract.
Results: The average age of the patients was 30.49±8.42 (21-51) and the mean follow-up time was 13.3±2.6 months (8-18). Preoperative visual acuities were light perception in 31 (51.6%) cases, between hand motion and 0.05 in 19 (31.7%) cases, between 0.1-0.15 in 4 (6.7%) cases and between 0.2-0.4 in 6 (10%) patients. At the last examination BCVA was 0.8 and above in 8 (13.3%) cases, between 0.5-0.7 in 11 (18.3%) cases, between 0.2-0.4 in 14 (23.3%) cases, between 0.1-0.15 in 13 (21.7%) cases, between hand motion-0.05 in 13 (21.7%) cases and light perception in 2 (3.3%) cases. In the early postoperative period; there were 10 (16.6%) cases with anterior chamber fibrinoid reaction and 4 (6.7%) cases with elevated intraocular pressure who responded the medical treatment. In the late postoperative period; there were 5 (8.3%) cases with macular scar, 4 (6.7%) cases with corneal opacity, 3 (5.0%) cases with retinal detachment and 2 (3.3%) cases with phthisis bulbi.
Conclusions: Pars plana vitrectomy is an effective and reliable method for removing intraocular foreign bodies. At the same time, preoperative and postoperative complications are important for a good visual prognosis and anatomic success.
