Surgical Extraction of Idiopathic Peripapillary Subretinal Neovascular Membrane

Case Report: In a 30-year old woman who presented with loss of vision in the right eye during past 3 months, subretinal choroidal neovascular membrane (SCNVM) starting from the temporal side of the papilla and extending to the juxtafoveal site with accompanying subretinal hemorrhage in the temporal, inferior sides of the SCNVM was detected. No associated ocular or systemic diseases was found in the patient whose visual acuity was 0.1. As a result of clinical, angiographic and optic coherens tomographic (OCT) evaluation, SCNVM was found to be suitable for surgical extraction and pars plana vitrectomy with posterior hyaloid seperation, extraction of subretinal membrane and C3F8 gas endotamponade were performed. In the patient who was followed-up for 6 months postoperatively, visual acuity increased to 20/25. Retina pigment epithelium (RPE) atrophy in the papillomacular region was observed with no SCNVM recurrence. In the postoperative OCT scans, RPE atrophy in the site where membrane was extracted was demonstrated as increased hyperreflectivity, whereas normal inner-outer photoreceptor complexes and RPE layers were present in the subfoveal region. Surgical extraction of idiopathic SCNVM can result anatomical and functional success in selected cases.
